If you are in at least one of the following categories:

1. You are 65 or older and entitled to a social security retirement benefit as an insured wage earner (at least 40 quarters of coverage (QCs) or fewer depending on date of birth).  

2. You are 65 or older and you or your spouse had Medicare-covered government employment.

3. You are 65 or older and you are certified by the Railroad Retirement board as a qualified Railroad Retirement beneficiary.

4. You are under 65 and you have received Railroad Retirement disability benefits for the prescribed time and you meet the Social Security Act disability requirements.

5. You are under 65 and have received Social Security disability benefits for at least 24 months.

6. You are under 65 and have End-Stage Renal Disease (ESRD) or Amyotrophic Lateral Sclerosis (ALS, commonly known as Lou Gehrig's Disease).

Must meet all of the following requirements:

1. Age 65

2. A U.S. citizen or lawfully admitted alien (5-year continuous resident in U.S.)

3. Must elect Medicare Part B coverage

4. Must enroll for premium Part A during the prescribed enrollment period

5. Ineligible for Part A under any other provision

Part A premium = $423.00/month (for individuals with fewer than 30 QCs) in year 2008
